Applying for an OMMA Medical Card
Qualification
Ensure you meet the eligibility criteria, including being a resident of Oklahoma and having a qualifying medical condition.
Physician Recommendation
Schedule an appointment with a physician registered with the OMMA. Obtain a signed recommendation form.
Application Submission
Complete the online application on the OMMA website, including the physician recommendation and required documents.
Payment
Pay the application fee online.
Processing
Wait for your application to be processed. You will receive an email notification once approved.
Card Receipt
Receive your OMMA medical card by mail. The card is valid for two years from the date of issuance.
